We are excited to announce that Weber School District has partnered with Intermountain Health to pilot a School-Based Telehealth program at Sand Ridge Jr. High. Student’s at Sand Ridge will have convenient and affordable access to a healthcare provider through Intermountain’s Connect Care during school hours. 

How Does It Work?

With parental consent, students will be able to participate in telehealth appointments with a licensed healthcare provider during the school day, coordinated by our school nurse. The appointments will take place using a secure video platform, ensuring privacy and confidentiality. Parents have the option of joining virtually and will be informed of diagnosis, recommendations, and any follow-up care if needed.

  • Seamless coordination between parents, school staff, and healthcare providers
  • Convenient and confidential care, delivered in a familiar environment

How to Get Started:

To opt in ahead of time or to schedule a visit, please reach out to Madi Smith at masmith7@wsd.net or call the school office. Parents/guardians will need to sign a consent form and provide insurance information. Additionally, verbal consent from a parent/guardian will be required at the time of each visit. All visits will be coordinated by contacting the school nurse.

What Does it Cost?

Most insurance plans are accepted. For families without health insurance, a Connect Care visit costs $69 and charges only apply if your student is able to be treated. Financial assistance is also available. Check out the attached flier for a list of insurance plans currently accepted by Connect Care. You can also check with your insurance company to see if Connect Care is covered by your plan and the costs associated.

Please know that our intent with this program is NOT to have students come to school sick. If students do not feel well enough to be at school, they should stay home. However, telehealth can be a great option for other minor health concerns. School-Based Telehealth Services have been shown to reduce student absenteeism and make healthcare more accessible and affordable for families. This program will enable students to access care without leaving school, ensuring minimal disruption to their education. We are excited to offer this valuable option to your family!
