On Thursday, October 28th at 9:00 am your school and the Weber-Morgan Health Department are partnering to provide their annual Flu clinics. In addition to Flu vaccine WMHD will be offering the required 7th grade immunizations for students in the 6th grade (Tdap, Meningococcal and HPV).  It is recommended that everyone 6 months of age and older receive an annual influenza vaccine to prevent illness and hospitalizations.  School nurses and Weber-Morgan Health Department will be offering immunizations to all students and school faculty.  Parents and siblings are also welcome to attend and receive vaccinations at the same time. 

 

In past years, each school has sent the immunization forms home with the students.  This year we are sending them electronically for you to print out and send with your child to school on the day of the clinic.  Each school will also have printed forms for those who do not have the ability to print them at home.  Click Here for the influenza form, 6th grade form, and clinic calendar showing the days and times for each school immunization clinic. The immunization clinics do not have an end time, but will be provided for at least one hour and only longer if needed to provide immunizations to everyone interested.

Important info: 

  • Pfizer will be the vaccine administered. A prime dose (first shot) and boost dose (second shot) should be scheduled, three weeks apart. 

  • Vaccine appointments may be scheduled at any of our secondary school sites. Individuals administered the first dose will receive a CDC card and will need to bring it to the site they schedule for their second dose three weeks later.

  • Students (ages 12 years and older), parents and school employees are eligible to receive the Pfizer vaccination at our school sites.
  • Health care personnel will be wearing masks in the vaccination area to protect themselves and others. For their personal safety, these health care professionals will ask that those coming to receive a vaccination wear a mask or face covering during the process.
  • The parent/guardian of the student must provide permission for the student to be vaccinated if the student is under 18 years of age.

July 9th: Parents can Login to their Parent Portal to make changes to Registration forms and sign all of the required online documents for the new 2021-22 School Year
 
July 19:  Parents can login and pay Registration fees
 

Aug 5th: Schedules will be available in the Portal if required fees are paid
