On Thursday, October 28th at 9:00 am your school and the Weber-Morgan Health Department are partnering to provide their annual Flu clinics. In addition to Flu vaccine WMHD will be offering the required 7th grade immunizations for students in the 6th grade (Tdap, Meningococcal and HPV).  It is recommended that everyone 6 months of age and older receive an annual influenza vaccine to prevent illness and hospitalizations.  School nurses and Weber-Morgan Health Department will be offering immunizations to all students and school faculty.  Parents and siblings are also welcome to attend and receive vaccinations at the same time. 

 

In past years, each school has sent the immunization forms home with the students.  This year we are sending them electronically for you to print out and send with your child to school on the day of the clinic.  Each school will also have printed forms for those who do not have the ability to print them at home.  Click Here for the influenza form, 6th grade form, and clinic calendar showing the days and times for each school immunization clinic. The immunization clinics do not have an end time, but will be provided for at least one hour and only longer if needed to provide immunizations to everyone interested.
